Jennee Swartz5 StarI just received my one year chip from Clean Slate in the Gilbert office. I've been going there through since July 2020. Since day one they have been amazing! Joby and Matt have been the best! They are a huge part of why I am still sober! All of the support I've received from Joby, Matt, Lora, and Ray I don't believe I would have found anywhere else! Leaving all of them at Clean Slate is the hardest thing I've got to do and I'm so sad to not be able to see their smiles every two weeks anymore. Thank you for the support, the actual caring, the encouragement, all the FLEXIBILITY with my appointments and the love! I would recommend this location over and over again! I'm not the only one they are like this with. During appointments in the waiting room I've seen and heard many patients come in and be greeted by Joby or Matt and every single time the patient's faces lit up and they seemed excited to share how their days are going and catching up since the last time they'd been in. Larry Walters1 StarI have been meeting with their providers for several years and things started out great but then became a pain and continued to get worse and worse. My latest left me hanging out to dry with out medication for a few days. She lied and had told me one thing while telling others (her boss, pharmacist, staff....) Something else. I had problems where the prescription didn't get sent from the managing Dr to the pharmacy on Fridays and there was no way to get ahold of anyone until Monday morning again leaving me without medication.Monday 10th January 2022

Shane Peters5 StarClean slate saved my life. I've been on opiates for 6-7 years and when I first got out of detox I didn't know where to go. Someone at the detox recommended clean slate and not 24 hrs after release from detox they got me in an appointment. Everyone is so kind, loving, and they actually try to get to know you and your story. Everyone genuinely cares about your recovery and health. Clean slate has been the backbone of my sobriety and without them I know that the 18 months clean that I currently have would not have been a possibility.
